I hereby agree to the following and represent and warrant that I am in good physical health and do not suffer from any medical condition which would limit my participation in the classes offered at Forma Strength Ltd (herein, “Forma”):

1. That I am receiving and participating in gymnastics and fitness instruction, offered by Forma during which I will receive information about and instruction about gymnastics, fitness and health. I recognise that classes will require physical exertion, which may cause physical injury, and I am fully aware of the risks and hazards involved.
2. I understand that it is my responsibility to consult a physician prior to and regarding my participation in classes. I represent and warrant that I am physically fit and that I have no medical condition that would prevent my full participation in all classes.
3. I understand the risks associated with the activities offered by Forma and I agree to follow all instructions so that I may safely participate in classes, workshops, or other activities.
4. In the event that I am pregnant, I will not participate until I have discussed the potential risks to me or my unborn child/foetus with my obstetrician. I agree that I will follow my obstetrician’s recommendations and on behalf of myself, my heir, spouse or other interest part hold harmless Forma for any possible injury to myself or my unborn child/foetus. I further acknowledge that pregnant women should not practice any inversions, lifting, twisting or any abdominal tightening exercises.
5. I understand that any statements made about my health are merely speculative, and that representatives of Forma are not licensed health professionals (with the exception of Wesley Tan and Claire Berthier, who are both registered Osteopaths). Therefore, in the event that I have genuine questions regarding my health, I will consult a physician.
6. In consideration of being permitted to participate in the classes and instruction, I agree to assume full responsibility for any risks, injuries or damages, known and unknown, which I might incur as a result of participating in the program.

I have read the above release and waiver of liability and fully understand its contents. I am legally competent to sign and voluntarily agree to the terms and conditions stated above.
